As a Reablement support worker you will contribute to a team approach in providing integrated multi-disciplinary programmes of Reablement across 7 days a week, 365 days a year. The role is aimed at promoting independence and helping to restore/maintain the service user’s ability to remain within their own homes. You will be able to demonstrate good interpersonal and communication skills and the value of promoting independence. You will be required to have a current valid driving licence.

As the service operates 7 days a week including bank holidays you will work on a rota basis which will include bank holidays, weekends and out of hours.

Your role:

We are looking to recruit Reablement Support Workers to join our team, your role will be to support people to live as independently as possible through encouragement, specific techniques and the use of aids and adaptations.

You will be part of a team providing physical and emotional support with tasks such as moving and handling, personal care, meal preparation, support with medication and mobility practice. You will also encourage people to engage with their communities and promote their health and well-being.

We will be looking for you to possess or have a commitment to complete an Adult Lead Care Work Level 3 credit value in Health and Social Care, Care Certificate within an 24 month timeframe and the Trusted Assessor Award. We will also be looking for a commitment to Equality and Diversity and respect for the rights of service users to privacy, dignity and confidentiality. You must be prepared to work flexibly to include some weekends/bank holidays and evening work.

The service can be provided for up to six weeks and is available 365 days a year. The team will support people who have just left hospital or following significant changes in a person’s ability to cope at home.

Enable and support people to regain confidence, ability, and necessary skills to live at home.
Motivate people where their ability to improve is possible.
Enable people to remain independent with the use of adaptations or assistive technology.
