Salary: Grade 6, £22,462 - £25,295 per annum.

Your role:


You will ensure the delivery of a high quality and effective service within the Integrated Discharge Team to ensure patients are discharged from hospital in a safe and timely manner. Working alongside a range of professionals to provide an integrated approach to service delivery, your role will involve assessing people’s needs, supporting them and their families at difficult times in their lives, making referrals to services such as reablement, moving and handling, care agencies or residential care homes. You will manage a small caseload of cases and will be expected to provide regular updates to other staff and managers.

We are looking for a highly knowledgeable and experienced individual, who can support the service user throughout their journey whilst in hospital, acting as their advocate, enabling them to speak up for themselves and represent their views accurately. You will have the ability to work flexibly and to work as part of a team to deliver an outstanding service. You will be able to work under pressure, respond effectively to emergencies and work effectively with families and health care professionals. You will have a positive outlook with a caring and compassionate disposition, be a good listener and a great problem solver.
