Salary: Grade 9, £32,029 - £36,876 per annum.

37 hours per week

 

Salary: Grade 9, £16,015 - £18,438 per annum.

An exciting opportunity has arisen for 2 Social Workers to join the integrated discharge team in based within the Wigan area. Applications may be considered from both newly qualified, Assessed and Supported Year in Employment (ASYE) and experienced Social Workers.

 

Your role:

 

You will ensure the delivery of a high quality and effective social worker service imbedded within the Integrated Discharge Team, to facilitate, empower and promote independence, strengths and assets of both individuals and local communities.  You will work alongside a range of health colleagues and partners to provide an integrated approach to service delivery. *Your role will be specific to supporting individuals and families through difficult times whilst ensuring that vulnerable adults are safeguarded from harm. Working within organisational policy and procedures you will be employed in a statutory position with adherence to the current legislation.  You will work with the individuals by helping them to utilise their existing skills, knowledge and relationships and enabling them to build on their strengths so that they can achieve positive outcomes.  You’ll manage a complex and varied caseload and will chair various meetings as required.  The focus on this role, will be to improve outcomes in people’s lives, maintain professional relationships and provision of assessments appropriate interventions to aid safe and timely discharge resulting in optimum recovery.  You will be expected to undertake safeguarding enquiries, participate in multi-disciplinary meetings and complete evidence based mental capacity assessment.  Additionally, the role also encompasses the management of complex family dynamics often at crisis point, complex case management; inclusive of cases with a mental health component.

You will be an enthusiastic and committed practitioner, capable of building professional relationships with service users their carer(s) and families.  You will be an excellent communicator who understands the value and importance of integrated working with partner agencies and the community.  You’ll be efficient in time management, report writing and completing assessments and a strong team player who is used to working in a person-centred way.  You will have a holistic approach to work with a keen focus on reducing inequalities and supporting individuals and/or their carer(s)/families to develop their resilience and improve their outcomes.  You’ll have a positive outlook with a caring and compassionate disposition, be a good listener and a great problem solver.
