Cardiovascular Disease Pathway Facilitator - Acute

Grade 9 - £15.95 to £18.37 per hour

37 hours per week

This post is fixed term temporary for one year

Your role:

You will be responsible for leading on the design, implementation and management of the introduction of key health improvement interventions within pre-operative assessments and targeted Cardiovascular disease and respiratory treatment pathways. 

Interventions include NRT prescribing and dispensing, fuel poverty assessments and rehabilitation focused physical activity.

This work has potential to make a significant contribution to reducing recovery time, reducing readmissions and support patients to manage conditions more effectively.

You will do this by building effective partnerships with stakeholders across the Acute and wider to accomplish the necessary adaptation or transformation of existing systems, processes and work practice.

You will be an enthusiastic and committed practitioner with the ability to build professional relationships with key stakeholders.

You will understand the value and importance of transforming systems and processes to improve health and wellbeing outcomes of our residents.

You will be an excellent communicator with strong project management skills as well as a strong team player who values the strength of working collaboratively for success.

You will be courageous and eager to try new ways of working and have the ability to influence other and have a strong understanding of the Acute setting.
